Universal intake
Three-stage assessment: program selection, universal core (demographics, medical history, PHQ-2/GAD-2/C-SSRS safety screen), and program-specific clinical module. Universal Safety Screen is non-negotiable across every patient regardless of program selected.
Pathway assignment
Based on your screening results, the platform assigns you to one of three pathways. Green: physical optimization. Yellow: physician review with referral to appropriate care where indicated. Red: warm handoff to specialty care, no Mon Élan enrollment.
Membership confirmation
You select your subscription tier — Foundation $179, Optimize $349, Concierge+ $699, or Enterprise (variable). If you select a tier below your clinical recommendation, you'll see a plain-language disclosure of what's included at the recommended tier and acknowledge it before enrollment. Upgrade anytime.
Provider visit
Video visit with your prescribing physician or NP. Lab review, protocol design, prescription routing. Your clinician sees your complete intake; you don't have to repeat your story.
Baseline labs
Comprehensive baseline panel ordered immediately. Drawn at a contracted lab partner location near you. Results return to your dashboard in 3-5 business days.
Ongoing care
Async messaging with your care team anytime. Follow-up labs at week 6-8, then quarterly. Therapy sessions and dietitian visits per your tier. Insights Program data flows into your personalized dashboard.
